Can someone tell me what is needed for the RWJ MD/PHD secondary?

All they sent me was the secondary (so called) for the regular MD.

i.e.

Are there essay questions and why don't they send me this information?

I found the MD/PhD application on this website: http://www2.umdnj.edu/~gsbspisc/mdphd.html

I filled in this form and mailed it to the address listed at the top, but haven't heard a thing from UMDNJ yet. (It's been almost 4 months 🙄. )
