I have a theory that either Covid and/or the mRNA vaccines can cause Sacroiliitis. There’s a few case reports.

Anyone seen a clear increase in SI pain cases in the last few years?

I think being in an inflammatory state makes joint pain worse. Both with covid and other things like pneumonia, the flu, etc my patients complain their back and knees hurt worse.

All pain worse with stressors. Pandemic, change in weather, loss of job, etc.

We probably should make distinctions between SIJ pain vs sacroilitis with mri proven inflammation.
Tell ppl this all the time, if you take the physiologic stress of chronic pain and add to it a bankruptcy, divorce or the flu, expect everything to get worse.

My 80 yo women with blow out lumbar spines who are caretakers for husbands with metastatic CA always blame the caretaking on their worsening pain status. That certainly contributes, but so does the stress.

you cant see 'inflammation' of mri of lumbar /cervical. What are you expecting to see on a mri of the SI joint...

The true difference between sI pain and SI inflammation.
 
Rheumatologists get their panties i a bunch over the nomenclature bc sacroiliitis usually means some sort of rheum syndrome. SIJ pain can exist without sacroiliitis.

You can definitely see sacroiliitis on xray ct or mri.

Seems like a whole bunch of you havent really been payong attention.

And no, there isnt more 'sacroiliitis' since covid.....

Well I think SI JOINT PAIN and DYSFUNCTION is a clinical diagnosis. Need to do diagnostic injections.

Different perhaps for sacroilitis.

This is similar to facet joint pain. There's no MRI findings. The test is the MBB.

i agree.

although there usually is some facet findings on imaging. SIJ is a different animal altogether. anybody who tells you they understand the motion/mechanics of the SIJ is probably lying to you
 
Except some insurances will not pay to treat sacroiliac joint dynsfunction but they all pay to treat sacroillitis. So you can guess which diagnosis I give my patients.
This is what drives me nuts about ICD-10. What’s the point of coming up with all the fancy ways to describe something, if none of them get paid.
