MD School List Help: GPA 4.0 MCAT 521 but...

Hello SDN,

Some things about me:

• Cum GPA: 4.0
• MCAT: 521
• Good amount of volunteering (~400 hours)
• A bit of research (400 hours)
• Shadowing: Only 15 hours, mostly with family practice
• Employment: Server at a restaurant for three years, over a year of CNA, and starting to scribe full-time for my gap year (I will also continue to CNA).
• Extracurriculars: Student orgs, committees, TA
• Schools to which you are applying: Regional schools, Baylor, Mayo, Arizona, Wisconsin ...Still completing this list

I know my stats are pretty good and my experiences are OK, but the reason I am creating this thread is I received an alcohol violation as a freshman, followed by being arrested and receiving a deferred imposition (charged but not convicted after a remediation period). I listed this in my institutional action section of my application.

Should I apply to some top-ish schools with this blemish on my application? I'm currently looking at a lot of OOS friendly schools from a wide variety of 'tiers' (if there is such a thing). Any personal recommendations from y'all would be appreciated! Thanks for your time!
