I can't recommend an MD application with a 499, unless you are a good candidate for the Puerto Rican schools, a mission-based school or a resident of a very lucky state (e.g. MS, WV).

These are DO schools where you have a chance for an interview with your stats:
ARCOM (new school)
ACOM
ATSU-SOMA
BCOM (new school)
WCU-COM
LMU-DCOM
UP-KYCOM
WVSOM
LUCOM
VCOM (all 3 schools)
NYIT-Arkansas (new school)
RVU-Utah (new school)
UIWSOM ( new school-Texas)
Many of these schools have MCAT medians for admitted classes in the 498 to 502 range and I expect the new schools to be in the same range. If you apply to DO schools you should apply this month and submit all your secondaries by September. You may need a DO LOR for some schools. You have a good GPA which is certain to attract attention at some schools.

RVU has a 500 cutoff - wont send you secondary unless you got a 500. LUCOM I believe has the same deal. Just FYI

If you want to have a better shot at MD I highly suggest you take a year off, spend some time studying, retake MCAT and apply.

LUCOM has a 500 cut-off so scratch that off as well. But yes, you have a good chance at schools considering experiences are fantastic.
