I did not match in the ACGME for an EM position. I may be forced into a TY - will I be able to match into a PGY-2 position next year? And does it matter if the TY is AOA or ACGME?

As far as I know, you cannot begin an ACGME advanced residency until you have completed an ACGME internship. I would stick with just the ACGME TY's.
 
I am in an ACGME advanced program after completing an AOA transitional year.
 
i know other people who did AOA years before entering ACGME programs. The key is to be sure to fulfill your required rotations.

I think the OP is asking if he/she will be able to finish residency in 3 years, being board certified in ER, if the OP starts out by doing a TY (either ACGME or an AOA one). I would think the answer is no...although I think the OP should check with the American Board of Emergency Med (isn't that the accreditation body for ER docs?). I don't know about the AOA vs. ACGME TY thing...although I'd think it will be hard to a get an ACGME TY in the scramble...usually those fill up with competitive candidates, like people doing rads or derm. If there are any spots left open, would think they would be at a less desirable program (i.e. one that doesn't have many electives, etc.).

There probably are some ACGME internal medicine prelim spots that are still open, that might be decent and that the OP could likely get...a prelim medicine year wouldn't be great, but I'm sure the OP could get SOME credit for it later if the OP switches to ER. Most medicine prelim years would include at least one MICU month, and often an ER month, too. Of course some of the IM ward months would count for the OP as well. The advantage of doing an IM prelim year vs. a TY would be that the OP is 1/3 the way through an IM residency, if things fall through and the OP isn't ever able to get into ER (sorry to be harsh, but it's worthwhile to at least consider the possibility).
 
AOA TRI, ACGME prelim IM, surgery, and TY single-year internships do not start the 'clock' in regards to funding. So 1-3 programs will get full funding even if you do an intern year somepace else.
 
I did a TY year and then matched into a PGY-1 ED residency- I suppose you could get into a Pgy-2 Residency for a 4 year program. It is doable. The TY program doesn't start the funding clock- and if your lucky you do the TY year at a program you would like to eventually match in.

Why not find an internship that is dually accredited? I know they exist. Just be careful with AOA programs. Because of the restructuring, there are not many PGY-2 spots. I have two friends that did TYs this year, in hopes of applying to our EM program as PGY-2s. Because of the lack of advanced positions, they could only apply through the match, and basically repeat their internship (basically a 5 year program). One actually matched at another hospital, and is repeating her PGY-1 year, only to have some of her internship count as credit off the end of her residency.

Either way, it will be 4 years. I know of one "dually accredited" EM program that really isnt. It's a 1-3 ACGME program, but to make it AOA approved, their matched DO applicants do an Osteopathic Internship, and then start as EM-1s while being PGY-2s with the Allopathic first years.

It might be possible that if you do a prelim ACGME year, you can get credit towards a 1-3 program, but most likely still won't graduate in 3 years (maybe 3 and 1/2)
