This could have some use as well for really sick vented focal pneumonia though that is uncommon and it remains to be seen how effective secretion clearance is. I was sad they didn’t show the underlying device allowing selective ventilation in the video or how it is placed. I can imagine if it takes an hour to intubate and isolate each lobe it would be of very limited utility

Right but how long will it take to place? How will you know it is actually working correctly? There is also the problem that a subset of patients dont have complete fissures and you might not be isolating upper/lower lobes completely so you would have to review an appropriately protocoled CT scan prior to the procedure to make sure they are appropriate candidates.
