Sending transcript to AMCAS requires a form with transcript?

Do you need to have the transcript form sent with your transcript? Or is it optional?
 
I don't believe it's necessary. Several of my schools only do online transcript requests, and so I wasn't able to have the AMCAS form included. Everything seems to have worked out anyway.
 
You don't have to have it, but I believe AMCAS says that not including it might result in significant delays or something to that effect. Not sure if it's an empty threat, but if you can include the form, I'd rather be safe than sorry.
 
My university has an online transcript request too, but they allowed me to fax it into the office and then sent out my form. If you want to call your office you can see if they will allow you to e-mail it or fax it in too.
 
My school had an online transcript request form, but I still faxed them in the AMCAS request with my signature.

They will know what to do with the form.
 
I called amcas about this last year. The form is completely optional. However, under alternate IDs, in amcas, you should list your school ID #. That will help then associate your transcripts with your file, if there is any confusion. I didn't experience any delay.

I am having transcripts sent from 3 schools - they all have my SS, but I guess I should write down the ID numbers for those schools as well?
