So I know both CMOs at my place of employment and volunteer gig. Is it not advisable to shadow either of them in these settings because of the additional duties that they may have beyond seeing patients?

Yes; they both see patients regularly

I see no reason not to shadow them at a clinical site during patient-related activities.
