Shadowing certificate

Hello again. I have shadowed under an orthodontist for about a month and a half. I have to get shadowing certificate signed by him. Can somebody please drop format of the above mentioned?
Any help will be appreciated.

I do not understand your question. .
Do You need the format in which ur certificate should be issued?
As far as my certificate goes. It was by my dentist's office only.
Basically it should be on their office leeter head and should have dentist name address, email and phone no.
And days/ month u hav worked wid him. And what did u do there and how were in doing those things.
Something about ur sincerity and ur learning skill.
This was pretty much on that.. hope i helped.
