Shadowing forms?

I'm shadowing several doctors over the summer and I am wondering if there are any AMCAS forms or anything that we need them to sign for verification of our hours.

Nope. You have to list the name and number/email address of a contact who can verify that you did the hours, though. Schools usually don't call the contacts unless it seems fishy.
