I keep reading on SDN that 50 hours is the target/min hours for shadowing and also some hours should be with a primary physician. How much does the primary doctor requirement count? What if I do not have a primary doctor but have shadowed in the ER (2 different hospitals), a wound specialist, and internal medicine?
 
Internal medicine can be primary care. Keep in mind that 80% of physicians are in primary care. You should have an idea of what that very likely path might look like for you. Office based internal medicine, office based family medicine, or office based pediatrics would be something to seek -- at least 8 hours.
