shadowing/researching hours report

I am almost done shadowing and researching for this summer vacation.
My last day for both researching and shadowing is on Wednesday.
I heard that some dental schools want to get the document along with the signature of a dentist whom I shadowed for and that of a professor whom I involved in a research with.

Is there a specific form for it?
Could you people give me an example?

p.s. Do I have to write what I've done or just report hours?
 
I was only asked by one school to verify my shadowing hours, that was Loma Linda - and they had their own form,all of the other schools took my word on it, and the same for my research hours. Now, it's very good to get a letter of rec from a dentist, and some schools do require that.

There is a small spot that asks you what you do on a day to day basis, it's really not looking for much detail.
 
Oh..... But, I thought that quite a few schools 'required' some of shadowing hours... like Tufts, NYU, and so on... don't they?

If you send your shadowing/researching hours report to Loma Linda, did you use a microsoft word to do it?

Many schools do require a certain number of hours, but they will most often take your word for it. I didn't apply to Tufts, but NYU never asked me for anything to prove it. The thing for Loma Linda could be just mailed in after you got the signature - and it was just shadowing, didn't mean to make it sound like you needed that for research too.
 
Are you getting an LOE from this dentist? If so, ask him to state the approximate number of hours you shadowed him in the LOE. Schools, such as Tufts, require a dental LOE (plus they specifically state that the dentist should include the number of hours you shadowed, which should be at least 30 hours).

Yeah. I am trying to get a LOE from the dentist.... but she is not from the United States, so I kinda have to make my own to get a signature from her... what should I do??

Well if it's the LOE that you're concerned about, you can have her submit a digital letter, and if they need to send a form in, they can just fax it - that's what I did for my Loma Linda one, since my dentist moved away.

You mean you have to write your own dental LOE and she will look it over and make changes she deems necessary? She can submit the LOE electronically and it will serve as her "signature". I'm not aware of any schools at this point that actually needs a signature from the dentist to confirm hours on a form, but they may ask at the interview maybe.

Do I miss anything? should I add more things? any recommendations?
 
Maybe indiciate that it's general dentistry? Some schools want specifically general dentistry, but that may just be inferred from the fact that it's a clinic. Why are you making a form btw?
 
Well, I thought that even though many schools take my word for it, it would be just safe and easy to track on what I've been done. So, when the time comes, I will just write my dental experience without thinking how long, where, and when I had shadowed...

I kinda recalled my college apps process. I couldnt remember what I did in my freshman and sophomore year, so I had to put too much time on it.

So, I am trying to be more prepared 😛 since my GPA is low 🙁
