It's probably more beneficial to shadow doctors in the US if you want to become a physician in the US.

Your philosophy is incorrect. If you wish to practice in the U.S, shadow doctors here.

Shadowed primary care doc in private clinic for 4 months total nearly 75 hours; while overseas in a public hospital pediatrics and ENT for a total of about 100 hours over four weeks. My philosophy is that doctors are really the same overseas or here, is that true? Or should I do more shadowing to doctors in the US?
If those 75 hours of private clinic primary care doc observation happened in the US, then you're fine, and have more than enough hours. The overseas physician shadowing should be looked on as supplemental, as adcomms don't share your "philosophy" that they are the same as US shadowing. You can still list it, but if you do, be prepared to answer questions about how healthcare varies in that country compared to the United States or Canada.

Don't matter your philosophy, only the Adcoms. Just make sure your time was meaningful and you actually learned something.
