Shift splitting?

Anyone in a hospitalist group where split shifts are done “under the table”?

Two nocturnists 7p-7a, each take alternating admissions and crosscover is split evenly by units. Share RRTs.

But what is going on is an informal agreement for one nocturnist to work 7p-1a and the other 1a-7a. So you might end up with 10 admits over 6 hr period rather than 5 to do if both were on simultaneously.

Effectively the hourly pay is doubled for both persons with huge chunks of time saved.

How common is this..?
 
Do both stay in house for the duration? It sounds pointless to me since you are just working harder but also getting sleep messed up every other day.
 
We did it. But since most of the admissions come in before midnight, we alternate admissions before midnight. Then split rest of the night. 12-4, 4-8; unless you get slammed or codes. You can still alternate between those two blocks. At least someone gets some shut eyes, and sort of maintain a regular daytime schedule.

Usually works out well. It was the unspoken rules for the night guys.

No. One of us physically in house 7p to 1a, then go home. The other arrives at 1a and stays to 7a. The trade off of having the better hours (7p-1a) is that admissions would be busier during the first half of the night though.

This sounds like a violation of your employment contract and I would absolutely never do this. This essentially constitutes theft since you are being paid for hours you aren't working (it depends on language of contract and whether you are a private group or hospital employed).

They will find out eventually when something bad happens it is revealed that only one person was available to manage it. The nurses probably are wise to this as well.

Be careful...if “they “ figure out that 1 nocturnist all that is needed, one of you is losing your job.
