Yes, you need to add UCR extension. You took college-level classes for college credit at an accredited institution.
You must include every class you took. If these are all included in your UCR transcript, this will be sufficient.

If extensions are included on the accredited school transcripts they are are not transfer credits (not that it matters much).
Just wondering...for example, someone who took a summer class at a non-extension place would need both transcripts, even if it was transferred and showed up on the accredited school's transcript, but it's good to know that someone who took a summer course at an extension would not. It's a big distinction.
@gyngyn how do you think adcoms look upon courses taken through an extension school? Are they similar to how CC credits are viewed?
Until extension courses can be shown to be equivalent to standard courses they will be viewed with caution.
