Depends on what the issue is and what role it plays in you wanting to go into your specialty:
Ex: Witnessed parent die a long, painful death from cancer and now want to go into palliative care? It plays a role and I could see it as part of a personal statement
Ex: I was a heroin addict til last year and now want to go into addiction medicine?
Don't mention it...at all.
