I'm currently in the process of applying to fellowships. I'm applying to one I rotated at as a medical student. If I were to interview there, it would probably be my #1 choice. Is it a good idea to write a personalized statement mentioning my experience there as a pre-med and the physicians I worked with? One of them is the program director. I don't know if this would be seen as tacky or not.

You can write and upload any number of PS's (to ERAS at least). You can only apply one to any particular program.

To answer the OP's question, there are some indications for writing personalized PS's for residency/fellowship, but they tend to be indicated for more program-specific issues, not specifically for you, as the rest of your app should speak to that in some way.
