so for antipsychotic drugs that block dopamine, you can get EPS, TD, and NMS.
Is chronic EPS = TD??
So TD is one of side effects you get that is an example of EPS, right? I keep mixing up these concepts, can anyone help me out? Thank you.

As far as I understand, EPS are an evolving set of side effects starting with acute dystonia, and then progressing to akathesia, bradykinesia and finally tarditive dyskinesia (4 hr, 4 day, 4 week, 4 month...you know the drill. 😛)

So to answer your question, TD is a kind of EPS and it's more common with the high potency antipsychotics i.e. Trifluoperazine, Fluphenazine, Haloperidol. It's often incurable and so the only way to stop it from happening is to monitor the patient and stop/change the drug before the onset of symptoms.

NMS is an acute response to antipsychotics, so will happen within the course of days and is characterized by:
  • Increased body temperature >38°C (>100.4°F)
  • Confused or altered consciousness
  • Diaphoresis
  • Rigid muscles
  • Autonomic imbalance
Treatment for NMS: Dantrolene, D2 agonists like Bromocriptine

Ref: Wiki and FA

I thought the bradykinesia occurs within hours, meaning before the dystonia?
Thanks for pointing that out.
Erm..I'm not too sure to be honest. What I've said above, is quoting FA i.e. 4 week

http://www.ncbi.nlm.nih.gov/pmc/articles/PMC3004713/ This article says that it's an early onset symptom (no specific timeline specified)

whereas http://www.gcrweb.com/PsychosisDSS/pharm/pharmsections/Anti_Psy/phar_eps.htm says it manifests within 3 months of therapy. 😵

So, no clue.

As FA says it..thats the order clinically too..acute dystonia first...bradykinesia takes time to develop..
