signing up for step3

hi everyone
i hope all is well-- just a quick question-- i noticed that there is a form that appears to suggest that it has to be filled out by your residency program in order to allow you to take step 3-- is this required?-- it is not entirely clear to me.. Also, how exactly do you sign up for step 3? Anyone know? Please help. thanks in advance....

You need to go to the FSMB website in order to sign-up for Step III. The process will depend upon which state you are registering through. Some states require proof that you are in an ACGME accredited program, while others have no post-graduate training program requirements. I believe all states do require proof of identification through a notary.
