What do you call the program coordinators by when you see them on interview day/when you email them?

Is first name appropriate or you should say Ms./Mr. _____(last name)______

Obviously if they are young (under 40?) then first name, but I never know what the right thing to do is with older folks.

I would play it safe and refer to them by their last name. You never know who will be offended... After you match it doesn't matter.
 
I usually let them take the lead on this. First reply to those ERAS-noreply emails is Ms/Mr. Last name. Then whatever they use to sign their communications/introduce themselves as.

Agreed. Some of the people I know who have been least tolerant of students presuming they can be on a first name basis were young. In general you keep it formal until they tell you to call them by their first name. This is true in any professional setting, not just for interviews.
