simulating same day as a consult

Do we lose any charges if simulate the patient the same day as we consult them for non-emergent patients.

Yes, you lose the E&M code for the consult. Still we do it if clinically necessary.

That's my understanding as well and I practice the exact same way.

Since CT sims for IMRT are now bundled, I wonder if you can bill the E&M on a same day CT sim for an IMRT patient though? Not a common occurrence, but I have some out-of-town patients that I often sim on the same day as consult if no further work up, etc is needed.
 
Since Medicare only pays for clinically appropriate, medically necessary stuff... I guess a consult for an emergency patient is not medically necessary ;) Or, heaven forfend, Medicare wants MDs to see patients for free??
