Do we lose any charges if simulate the patient the same day as we consult them for non-emergent patients.

Yes, you lose the E&M code for the consult. Still we do it if clinically necessary.

I've always wondered if that held true for inpatient codes as well, never really bothered to check

That's my understanding as well and I practice the exact same way.

Since CT sims for IMRT are now bundled, I wonder if you can bill the E&M on a same day CT sim for an IMRT patient though? Not a common occurrence, but I have some out-of-town patients that I often sim on the same day as consult if no further work up, etc is needed.

Since Medicare only pays for clinically appropriate, medically necessary stuff... I guess a consult for an emergency patient is not medically necessary 😉 Or, heaven forfend, Medicare wants MDs to see patients for free??
