This theory that the "underclass" is going to be the demise of white collar work.

It's rather laughable.

If anything, it's because the white collar people decided to marginalize the middle class starting around 1970. It is the decimation of the industrialization of the US. "How they vote" is irrelevant. Rich people hold all of the lobbying power. The cash physicians don't get, the insurance companies and pharma companies get. Practitioners are easy targets because they aren't Pfizer. Just see what happened in Canada with pharamcists and generic drugs.

Blaming the poor people is just idiotic.

Political expediency makes careers in health care very susceptible to downward pressure on wages. It might not be a problem when the country is wealthy with a vibrant middle class who can pay their own way, but once the middle class tax base shrinks, good luck. My state in Illinois has cut hundreds of Medicaid programs because of its perpetual budget crisis, ranging from drug rehabs to early intervention for autistic children. At some point the consequences of the cuts will become visible on the street, and then what? The money is still not there. The overall macroeconomic health of America in the next half century will have a very large influence on careers in health care, they're certainly not immune to it, and are likely to face far more political pressure than just about any other white collar career.
