Temple ACMS is the only SMP program with guaranteed admission, provided that you score a 30 on your MCAT and maintain above a 3.5 in the program.
Tulane Anatomy Certification Program (ACP) offers something very near a guarantee, provided you beat the med student average in anatomy. However, you must be on a med school waitlist somewhere (D.O. counts, Caribbean doesn't) to even apply to the Tulane ACP.
End of list. These are the only two SMP programs that give you anything like a guarantee that you will get into a US MD program. They are both very hard to get into and have an applicant pool with rather high GPAs. If you aren't already competitive enough to get interviews (i.e. sort of competitive for med school), it is going to be hard to get into these programs (and impossible in the case of Tulane, because of the waitlist requirement). Also, financing these programs is difficult because they do not confer a degree, so it is tough to get loans for them, although Tulane, with a tuition of <20k, is comparatively cheap.
If you are an under-represented minority, there may be programs with guaranteed admission to med school for people with sub-par stats. I am not that familiar with such programs. Also, if you are interested in DO programs, there may be programs with guaranteed admission to DO schools. I am not a URM and my goal was to get into a US MD school, so I am not as familiar with those paths.
