Only on a pig in live tissue lab. As much as I enjoy some excitement, I hope it never comes to that in real life, as it will mean that several things have all gone horribly wrong.
 
I've done one in a real patient, and I'll never forget it. I always wondered if, when push came to shove, I'd perseverate on the things I knew how to do but weren't working, of whether I'd step up and do the thing I knew needed to be done, but that I'd never done before.
 
Once in 20 years. Went well. Patient left the hospital intact. Used the Melker kit.

Done it once with the CA-3 (I was a CA-2 at the time). Terrifying.

Witnessed one by the ED.
Witnessed one by ENT in the OR.

Know of 2 guys in our department that have done it, I think both were remote site intubations.

One of those things that you hope you never have to do...
 
Did it once on MICU
We couldn't ventilate. I had a perfect view of the cords. Passed the tube, but wouldn't go beyond the cords and still couldn't ventilate. Jet ventilator wouldn't pass either.
Patient bought himself a cric
