Guys question for you. It affects my residency decision so posting here. Can one have J1 extension for fellowship after residency by getting second statement of need from a different country than the country from where he got statement of need for residency.
Reason I am asking is because I am currently a permanent resident of one country and I am going to lose permanent residency during (medical) residency in US. Then only country I can go to is my country of origin and cannot go to previous country. Can I get a statement of need for fellowship from my country of origin in that case?
 
This is a very complicated question, and best directed to an attorney who specializes in this area.

One question to clarify -- if you do this, will you will have two HRR's to deal with? That will require two J waivers if you want to stay in the US.
 
yes that was my next question to ask.
ECFMG can entertain both residency and fellowship training. Once sponsored for a base residency and subsequent fellowship training requested would be considered an extension of sponsorship. A new statement of need would be required for the fellowship. Since J1 visa is of 6 years, both trainings will be covered under same J1 visa.
Now above is for someone who has same country of permanent residence for both cases. My situation is different:
I have been living in Canada as a permanent residence for the last 2 years and according to the process of obtaining J1 visa, I should be applying from Canada as this is my recent place of residence. However, during my medical residency, I will lose my permanent resident status in Canada as I won't be able to complete residency days requirements in order to maintain permanent resident status in Canada. Hence, I won't be able to do 2-year Home requirement as I won't be able to come to Canada as a permanent resident. I can return only make permanent residence after my residency in the country of my origin (country of my nationality). In this case, which country (Canada or country of my origin/nationality) I need to return to. It defeats the purpose of SON as the country that issued me SON cannot have me and I can only go to country from where SON has not been issued. I can get SON from my country of origin as well during residency to topple previous SON and then get another SON for fellowship. However, its complicated and it might jeopardize my chances to train in US.
 
Based on your name, I am assuming you are from India and you are currently Canadian PR- working in USA as a resident in a J 1 visa ??? Since you are about to loose your PR status - The next SON would be needed from the country you are supposed to return which is India. Another option is to try and maintain Canada PR and apply once you get your status back on track. Correct me if I am wrong, wasnt it a very expensive process to get your PR status? I know you will eventually settle in USA but maybe after fellowship. Till then isnt Canadian PR better than living in India?
