What happens to the student debt when trying to specialize? Does it keep accruing and do you have to keep paying or are you considered in school and dont have to pay until post specialty training? How do people manage going into specialties? Do you take more loans out?

Interest accrues, and if you are entering a residency with a tuition of course you have to take out more loans for it. That's how you end up like Dr Mike Meru, a negative millionaire.
 
Interest continues accruing but you don't have to pay it until after you finish your specialty program. Some programs pay a stipend like Peds and OMFS, but for most endo, ortho, perio programs you'll have to take out more loans. Some specialty programs can tack on another 100k+ of debt.
 
That’s why it’s important to do your research as well. I know of many programs that have no tuition and give residents a stipend of $50k+

Probably need much higher grades for those programs too.
