Off the top of my head, I'd say likelihood of potassium toxicity. Anyone with real knowledge want to chime in?

What is your source? Is it a relative or absolute contraindication?

There risk is as others have stated but beneficial effects of spironolactone on comorbids conditions has driven some research in this area. There are algorithms to direct rational and appropriate use in patients with CRF and compelling comorbidities but there is/was insufficient evidence generated for consensus. Maybe that is changed.
