Has anyone taken SRTA and had an automatic failure for open contact, yet didn't recieve any post-operative treatment form for their patient. I thought you were supposed to receive this form for an open contact; however, I had been told that some people don't receive this form, yet still fail for open contact and they just call the patients later and tell them that they have to redo the restoration......

Yes, this happened to several people in my class this year. Open margin/Short margin or open contact was the reason that was stated for the automatic failure. They never received an "instructions to candidate" form or a "post-op" form.
