Can we discuss stanford vs ucsf - any major differences in focus/quality/culture of the programs? How about research opportunities? Level of business? Any of them better in one subspecialty vs another?

these programs are totally different. I would say that when you apply, apply to both and see if you get interviews. you get a pretty good impression of the differences on interview day.
