State of Michigan Nitrous Sedation Requirements

Hey all,

To dentists practicing in Michigan, can you give me more information on nitrous oxide sedation certification and where to apply? The LARA website only has a nitrous oxide analgesia certification form for RDH and not dentists. I'm not sure where to apply, and how many total hours of didactic and total hours of clinical experience I need prior to graduating dental school.
