Best to ask places you're interested in. I don't recall that many applicants who didn't have Step II done, but doing well on Step I implies a likelihood of passing Step II in similar fashion. The places I was at were perfectly willing to interview someone without Step II but waited for their scores to pop up before ranking -- as I recall, anyway. If you just squeaked by Step I, then it's probably wise to have Step II passed to get that perceived hurdle out of the way.
 
On this point, UCSF definitely requires it (for everyone), for ranking (I don't know about interviewing ... it sounds like they do that without regard to StepII as KCshaw was saying).

I heard that UW, the Harvards, and Pennsylvania schools (and someone else Duke?) were also supposed to start requiring it soon (not sure if it was residency specific though).

As I understand StepII-CS would be more important to schedule earlier, as the scores take longer to come back. I would be interested to hear about the timing from people who have gone through it.

Last year I took a study/vacation month in July to get CK out of the way, but I wasn't able to schedule CS until February. Other than the occasional comment from an interviewer that my step scores made it clear that my academic skillz were more than good enough, no one really mentioned the exams in the interview process.

I had also heard the rumor that some schools were beginning to require CK scores to either offer interviews, or, more commonly, rank an applicant; since my CK score was already in by the time applications went out, the point was moot and I never found out which schools these were...sorry I couldn't be more helpful in that regard. I would definitely email/call the program coordinators of any programs you are particularly interested in to ask about their requirements if you're worried about it.
