I already posted something similar, so I apologize if I annoy anyone by posting about this again. The situation is that I am trying to decide if I should pursue the USMLEs anymore. I am on the fence about whether I passed or not. I am asking for feedback, and please tell me your honest thoughts based on how your exams went, and how you scored. Please be honest, as I not just looking for reassuring words. Thank you.
General mistakes:
- I didnt retie the gown for any of the patients
- I draped the patients incorrectly for abdominal exams. For example I pulled the gown down completely, exposing the chest for each one.
- I didnt summarize the H&P for anyone. I did a closure, and explained my workup, but never did a summary.
Mistakes per case:
1) Went well.
2) Wrote 'impinged nerve' rather than disk herniation for the main diagnosis. I completely forgot to do the spurling test. I also did not counsel her about something dangerous she had been doing, and ran out of time answering her second challenge question.
3) The patient was lightheaded. I did not have time to do any part of the Nuero exam. I only did heart, pulmo, extremities, then did closure. I consequently am not sure of my diagnosis. I forgot to include Tilt Table test in the work up even though i put orthostatic hypotension as a diagnosis. In hindsight, I realize I missed the diagnosis completely.
4) went well.
5) Another lightheaded patient. I again didn't have time to do any of the Neuro exam, and immediately went to closure. I had to guess about the diagnosises for it, and had to guess the workup for it. I also did a poor job answering her challenge question, in that I was constantly stumbling and starting over.
6) went very well.
7) went very well
8) went okay
9) I did not counsel the patient at all, and he had a lot of problems. Also, he said he was diagnosed as prediabetic years ago, but I forgot to ask him questions about it. I did not do a sensory exam on his legs, or do a fundoscopic exam. I believe this lead me to miss the 2nd diagnosis on the differential.
10) went well
11) I did not get the secondary diagnosis correct. I got the main diagnosis though. My closure for her was very short, and I did not answer her challenge questions well.
12) went well
Any feedback from those who already took the exam is much appreciated.
