I had a q...

was doing a q on USMLEWORLD. the kid had sickle cell and they asked whats the MCC of osteomyelitis in that kid.

Traditionally, if I recall S. Aureus is the MCC of OM in the gen population and in sickle cell kids. However, if Salmonella were isolated THEN the child was more than likely to have sickle cell.

However,......USMLEWORLD blatantly states that the traditionally wisdom of s. aureus being mcc of OM even in SCD pts is incorrect AND they sited a couple of evidence based articles.

Nevertheless is this true? They stated that the textbooks that still maintain that S. Aureus is the MCC in SCD pts are not correct. 😎

We all have problems with USMLE world being too detailed and uptodate. I would stick with the previous ascertion that staph aureus is the most common cause of osteo in non-sexually active people. Salmonella is more specific for sickle cell patients.

Hopefully they both will not be choices or they will give you more information about the gram stain.
