I have used practice materials from blueprint, uworld, aamc but still struggling in

Chemistry:
-Log based math (pka, pH, Ka)
-Molarity / titration
-Exponents
-pressure & volume

&

Physics:
-When to apply appropriate formulas
-Trouble remembering relationships between formulas/units

Any recs? Thanks!!!

If you're really struggling with these concepts, and review books and practice questions don't seem to help, it is usually worthwhile to use a textbook instead. It will likely take longer to read through a textbook, but it's excellent for developing a foundational understanding that isn't provided by review materials. For general chemistry, I know the Petrucci General Chemistry: Principles and Modern Applications is used in many courses, so consider borrowing it from a library if you can.

Also most bookstores sell QuickSheets with useful formulas...you sound like you are struggling with math more than science itself. It might be worth it to buy an algebra textbook. Just an idea.

For knowing which formula to use, make a list of knowns vs. unknowns.
Units will help a lot with that (match units to equation type/formula).

Other than that, Kaplan really is the best source. I annotated EK with Kaplan.
