UCSF has a form to request your undergrad file before matriculation, I don't know who else formally requests such information.

Depending on the transcript, they would/wouldn't want more information on what happened. Now if you weren't honest on your AADSAS application and your transcript showed misconduct, that would be a new issue.

Honesty is the key, they don't care if you did something dumb once. They want a competitive applicant to admit to any wrongdoing and show what personal growth attributed from said action(s).
 
My transcript has no records of misconduct at all. I have tripled checked.

Although, I asked my school to see whats in my student file. I have one violation.

Its an academic violation from freshman year. My friend and I worked on homework together and we both had similar answers for a question that was worth very little points and got marked with plagiarism. Would this kill my chances of getting accepted?
 
Of course not, your chances are based on how competitive your numbers are.

Even though this infraction is not on your transcript, admit to it on AADSAS. Talk about what you learned and explain your side of the situation. Plagiarism isn't taken lightly but given the right stats and overall application, it shouldn't immediately disqualify you. You need to take the higher road as you found being dishonest in the past kicked you in the butt.
