If one goes through the San Francisco match process (for say, neurosurg or ENT) and does not match, can one still enter the NRMP regular match, for, say, general surgery? Are there any disadvantages?--Is this like applying really late to med schools, in that fewer interviews are given out the later you apply for regular match?

I've heard of people double applying, but I don't think they wait until early match is done to apply. You would miss nearly all of the deadlines for applications if you waited until January 20 to apply.
 
Yes, you can apply to specialties in different Matches, just like you can apply for more than one specialty in the same Match. I would bet that most people entering the SF match apply to other specialties as well, given how competitive the SF specialties are. You have to apply, interview, etc, for both specialties at the same time. Yes, it's a pain in the ass, but it's the only way to really be successful. If you match in the SF match, you just don't submit a ROL for NRMP. You can't wait until AFTER the SF Match to apply to NRMP programs... it's too late.
