What are best resources to study Surgery for your shelves as well as for step 2? I've heard good thoughts about NMS Casebook but it is quite old. Is it really worth it?

NMS Casebook is very good, despite being old. Pestana notes are great for shelf-study, and includes subspecialties which aren't in the NMS Casebook. I used the Pestana notes to study for Step 2 CK as well, because they are more comprehensive for surgical topics than any standard Step 2 CK review book (like MTB).
 
The version of the surgery shelf we took was basically medicine. Uworld is a great way to study for Step II, but I found the questions in it were more "surgery oriented" (eg. do we take the guy with X history to surgery now or do a FAST exam first). The questions on the shelf we got seemed to ask more about medical management and pathophysiology.

There's lots of pathophys on the shelf exam, because surgeons need to know pathophys. There's a lot of medical management because surgeons do a lot of medical management. I don't get why people are surprised when these topics show up on the surgery shelf. Granted, some ridiculously bizarre questions show up on the surgery shelf, but knowing Pestana + UWorld + the general knowledge you've accumulated up to this point is enough to score well on the surgery shelf.
