This may be a bit off the beaten path, but here goes...I am currently a PGY-3 resident that initially matched into surgery. Well, about halfway through my internship I had a family member that was ill, and I thought of taking the year off....Instead, I found an open spot on the east coast and completed my internship on the east coast without missing anytime whatsoever. In another year I finish my residency, and I am wondering if this is legal...What if my initial PD does not write a letter of rec for me. SHould I check in with the ACGME??? Help, please.......

Shouldn't be an issue, but you are confusing me with your time line: You are a PGY-3 in surgery or anesthesiology? I'm assuming the latter. If so, if you have applied for and received a state license, you are safe as all state licensing agencies require certificates signed by both your internship and residency directors for licensing. Since you transferred mid year, it should be your east coast internship director that writes this and confirms satisfactory completion of your internship as they should not have graduated you from your internship year if they did not fully accept your months from the previous internship site. Also, the fact that you are a PGY-3 in and of itself confirms that you have satisfactorily completed your internship.

If there is any doubt about your internship, your residency should have/would have denied you permission to start your PGY-2 year.
