Switching. How can I make it happen?

Hi,

As a 4th year, I was torn between FM and EM, chose FM. Now I'm sure I made the wrong decision. I've considered the reasons why I'm ready to change careers and I'm comfortable with my choice. I'm just not sure how to do it.

I'm 6 months into a family medicine residency. No issues or problems - the people in my program are fantastic and very nice. I couldn't have asked for a better place to do residency. I'm just not happy with FM - the pace is too slow and I'm more interested in international/tropical medicine as a career.

I had strong board scores, excellent letters and great grades in all my rotations. I thought FM was for me, and it's really not.

Should I try to scramble this year? It's very important to me to be at a program with a strong international health component. Or should I just wait until summer, re-apply through ERAS for programs that have what I'm looking for?

And yes, I really am gorgeous😉

Do a search for Panda Bear. He has followed a lot of the route you are now on. His blog is a great read and map of what you might want to think about doing.

As for the pace being too slow, you're a first-year resident. If you want to bust your butt, you'll certainly have that opportunity when you're out in practice.

That being said, you shouldn't continue training in a field if you're truly unhappy. Before you switch, however, you'd better make sure you have solid reasons for going into whatever field it is you think you'll like better. You might be able to switch fields once, but probably not twice.

There are fellowships in international medicine for EM that are not available in FM. And yes, as important travel and international health are to me, that's one of many reasons for switching.
I realize all this, and I've decided that if I switch and remain unhappy, I'm leaving medicine for good. The fact is I am miserable right now, and it has nothing to do with being a first year resident. It has to do with the nature of FM and how I want to live my life.

And once again, I really am gorgeous😉

I've decided that if I switch and remain unhappy, I'm leaving medicine for good. The fact is I am miserable right now, and it has nothing to do with being a first year resident. It has to do with the nature of FM and how I want to live my life.

It's not like I'm pulling this decision out of the air. EM was a consideration last year.

Am I gorgeous? Well, let's put it this way: there's true tachycardia, and then there's what happens to patients when I walk in. We programmed our electronic charting with a little check-box that comes up whenever someone enters a high heart rate: "Feb present in room Y/N?"

Well, first we need to make sure you're a female, cuz if you're a male and you're telling everyone you're gorgeous, the thread count is going to drop precipitously.

I agree with Kent, you need to make sure its FM you don't want, instead of the thought process that international medicine is what you are looking for. Have you tried contacting programs/fellowships in international medicine to see if they take FP grads?

I would say that you can try and scramble, and see whats out there. If there are no programs you are interested in, then you can reenter the match. You'll have an extra year to deal with but you can make good use of it, either with travel, or other things.

I am sure you would do fine in the match but you will need to be prepared to awnser a lot of how do you know EM will be right for you? (and I am sure its a thread awnser but EM being a consideration isn't enough) No one wants to fill empty spots in their residency....

Well, by 'cat fight' I was thinking more along the lines of Foxy Boxing or some wresting involving either mud or jello.
 
Whatever. Women who keep insisting they're gorgeous, while they may well be earning due props for a healthy self-image, invite a little suspicion, with the apparent need to make a point of it.

Sort of like people who tell you what they drive within five minutes of being introduced, or people who not only know their IQ but seem to think the rest of us would care what it is. Or me, with my ridiculously high GRE score. It's a little off, somehow. Makes you wonder what's behind it.
