It seems like there's a lot of questions on this forum about transferring specialties. I've searched but can't find an answer to my question. I'm a PGY2 (soon to be PGY3) in a 3-year specialty. For numerous reasons, I'm considering switching to a different program in another state within the same specialty. I know they have an opening in their PGY-2 (soon to be PGY-3) class.

Is it possible to transfer and do your last year somewhere else?

Or is there some technicality that prevents this?
 
I imagine it depends on the specialty. For example, in surgery you must spend the last two years at the same institution, and the ABS website is very clear about this wth an easy Google search.

Try checking the corresponding information for your specialty boards.

The same is true for FM. I don't think it's true for IM, but whether or not you'll be allowed to graduate is completely up to the PD.
 
Step 1 find out if this is allowed for your specialty.

Step 2 find out if the other program's PD wants to fill that spot and would be willing to fill it with you.

Step 3 approach your current PD. Chances are if you have legit reasons for wanting to move they will help you, but obviously don't approach them unless you know this is a real option.

Also, review all the relevant NRMP guidelines. Technically, if you speak to the PD at a different program before requesting an NRMP waiver and get it approved and your PD at your home program finds out, this can constitute a match violation. If your home PD decides this needs to be reported, the NRMP can impose sanctions which delay you from advancing a year, or participating in the match for fellowship. Not fun. Also, if you are matched for a PGY-3 position, it would be a violation for the program you are interested in transferring out to discuss this with you as well, so they may not want to take on the risk.

Match violation only before you've worked 45 days. OP has worked 2 years, they aren't bound at all by the match any longer.

Sounds like a bad idea. What are your reasons?
