I understand for Synthroid the indications are to take on an empty stomach with a glass of water at least 30 minutes/1 hour before a meal. Do flavored drinks count as a meal? What if I patient wants to take it with a cup of coffee or with a soda or glass of orange juice? Sorry for such a noob question, please dont beat me up.

Food and drinks have the potential to decrease Synthroid (levothyroxine) absorption. While coffee by itself probably doesn't interact with the medication, the calcium in the milk or cream might. The significance of this interaction is minimal since the dose of Synthroid will be adjusted over time to make up for the small portion that isn't absorbed. Same deal with OJ. Advise your patients to take their Synthroid the same way each day.

Discourage your patients from taking their medications with soda and other sugary beverages to prevent NIDDM and obesity.

I think you hinted at it but...

at the same time if your patients have been taking their medication such as synthroid with coffee or food you do NOT want to discourage them from doing that. The kinetics of the drug will be altered if you go from taking your synthroid with bacon and eggs to not taking it resulting in a higher plasma concentration. Ideally when they are put on the medication they will be aware of this however **** happens.

thanks to both of you who responded. That makes a lot of sense. I appreciate it.
