Hi. I was wondering what you all use to take notes during clinical rounds and during preceptorships? I need a method that is easy to carry but will allow me to write legibly so that I can read my own handwriting later on. Thanks.

I carry a little notebook and scribble down notes of things I know I won't remember. When I then sit down to write the SOAP note, things aren't always in order but between my memory and the note all the info's there so I'm able construct the SOAP and think through the issues. As far as legibility since those are your personal note, you just have to make sure they're legible to you.
 
some guy that played a med student on ER had some earphone headset thing to talk into and record whatever he said.
