Test question: bleeding from external os?

I just took a test and one of the questions is really bothering me... Basically, it said bleeding from the external os is of greatest concern for...
I picked CIN. The other choices were endometriosis (I don't think that is right) and something else that was obviously wrong. It was meant to be a straight forward question, no history was even provided, but do you automatically think cancer when non-menstrual blood is present?

From my understanding of endometriosis, couldn't there be endometrial tissue on the external os that could be sloughing and bleeding? Did the test question specify that it was not during menstruation?

Please correct me if I am wrong, I am just a new med student.
