Yes we did. I still feel the need to give it to them. But instead of giving it to them all at once, I now give them 4mg prior to induction, then repeat 4mg before leaving them in recovery.

Are you doing this for all your pts? Just curious.

Probably 95% of the time. Most of my patients are women because the surgeons who use me are mostly OB/GYN's.

Also, I cant remember who suggested it to me....I think it was Plankton, 6.5mg of promethazine works well with zofran...no sedative effects noted. If I dont give it in the OR, I always write an order for the PACU nurses....although I wonder if they actually give just 6.5mg (kind of doubt it).
 
Right before peforming the spinal for a C-section, I often will give 30-50mg of propofol.

Oh I agree....it is very dangerous. But if it is done right....it makes all the difference in the world. You truly do have to size up the patient accurately. If there is any doubt at all, they get zero propofol. Obese patients...no way. Indian patients hate the buzz....so none for them. 🙂
