I was wondering if there is a title required for the OTCAS personal statement? Would you recommend writing a title? Also, do you put your name on top of the personal statement and what format did you use? Thank you!

Personally I didn't put a title, just personal statement.

Did you end up putting "personal statement" at the top? And did you end up putting your name?? I am about to submit mine and cannot find formatting info anywhere!

I just saved it as Personal Statement, did not type on the top "Personal Statement". Just saved it as that document
