TMDSAS: "Personal Bio" and Chronology Question

I am confused on what exactly they want us to enter here....

Personal Bio: What is the format for this?? The instructions say write dates, # of hrs ect.... But is this also where we write what we did in each activity?

Chronolgy of activities: Is it the exact same thing as personal bio but just in chronological order? Would we include joining organizations in this? Do we write what we did in each activity HERE (on the second line)

Also, if someone could please give me an example of the format for the personal Bio because theres no sample on the TMDSAS site!

I don't think we put what we did in either Personal Bio or Chronology of Activities except for a brief description in the latter of the two.
In the bio, I put, for example:
"Have you participated in any health-care related activities since beginning college? Yes No
If yes, list any health-care related community service or volunteer or employment experiences in which you have participated since you began college. List the inclusive dates and the approximate hours per week you spent in each activity."
One response I put in here:
"Pharmacy Technician from 07/05-06/07; 40 hours per week."
 
so theres really no where to describe what it entails? what about the second line in the chronlogy

Personally, I filled up the bio stuff with names dates and hours. Most titles and names of things are self explanatory (at least for them to have an idea of what you did), and you can elaborate about things you are especially proud of at your interview.

Chronology is there just to put everything you did in order. (redundant yes, but we do what TMDSAS requires). I usually had more room in this part and so I explained what I could. There are a lot of character limits so be concise.

Here's an idea of part of my personal bio: (some names changed)
List any leadership roles or positions of responsibility held while in college and to the present.

Super Duper Sorority, scholarship chair 8/2004-12/2005 (17 months), parliamentarian 1/2006 to present (1 year term); Dorm academic coordinator, 9/2004-12/2004 (4 months); Texas A&M Pre-Medical Society Chair, 5/2005-5/2006 (12 months); Collegiate Panhellenic Council Director of Academic Affairs 1/2006 to present (1 year term); March of Dimes Collegiate Council Vice President of Communications, 9/2003-5/2004


As opposed to part of my chronology which was more like this:
08/2004-12/2005:Scholarship Chair for Super Duper sorority, revised scholarship plan, met with ladies not meeting academic standards, 5 hours per week
