Is anybody having trouble typing in the addition information needed on the Transcript Matching Form? Can this just be handwritten?

I printed mine out. I'm filling it out by hand and mailing it to my registrar.

For "Dates of Attendance", are you putting up to this point or the expected date of graduation?

Mia, I put up to this past semester completed.

How about future classes? I'm enrolled in an online class, but haven't started yet... I don't want to go through the trouble of getting a transcript from them now if I'm not starting the class til the fall... any suggestions on that one? I guess I'll try asking their customer service... this is going to be a longer process than I originally thought!!

Is "Former Name" kept blank or "N/A" if its the same as the current name?
