do we HAVE to have this sent by the school with our official transcript?

I ask because my schools use the national clearing house to send transcripts so I dont know who to contact and tell them that they need to fill out this form and send it with the official transcripts.

Has anybody have their transcripts from their institutions sent to PTCAS? If yes, any delays?

You go to the enrollment people and give them the form to include when it goes through and they send it out

the school is in a different state....do I email it to them or fax it? would that work even thou we have to go through the national clearing house to order transcripts?
 
I was in the Army for more than a decade and I moved around a LOT. When taking into account a couple schools I took classes at after I got out I think the total was 11 or 12 schools. The schools were spread all around the country so submitting the forms in person was out of the question. A few of the schools only had the option to submit a request online (including at least one from the national clearinghouse) and there was no way to submit the PTCAS form. In those situations I made a notation in the remarks/additional info section asking them to include my PTCAS ID number or whatever it's called. I honestly have no idea if the included that info or not (the same can be said for my faxed requests), yet all of my transcripts arrived without any issues. If there was any delay it wasn't very significant, although I did request my stuff pretty much as soon as the application opened just in case there was a problem.

Call and ask. And yes those transcripts still get sent from your school so if you talk to them they can add it to the envelope.
 
Just fill it out and fax it to them with a note to include it in the sealed envelope with your transcript.
