I am just about to start my PGY1 year in internal medicine. I would like to transfer to a philadelphia program in internal medicine for my PGY2 year because of family reasons. It is very important that I do this and I am willing to do just about anything. Can someone offer some advice about Philadelphia PGY2 spots and what I can do? Any advice would be helpful. Thank you in advance.

1) Consider talking to your PD about it, but not right away. Feel him or her out first to see if s/he will be helpful. Some PDs will help you transfer. Others will actively block the process.

2) Consider transfer sooner rather than later (ie in the next month or two). This causes some problems with the residency, but it might be easier for your IM program to pick up a brand new person to replace you and there may be PGY1 IM spots in Philly.

From someone who transfered as a PG2 into a program here, it was very hard to adjust. Better to transfer sooner.

3) Directly email PDs in Philly and ask them about spots. Be honest about your situation. If they have an empty spot they'll be dying to take you. Then you just have to arrange it with your current PD, which may be tough.
